trouble logging out

I have two questions.

For one, just recently, I’ve been unable to logout on the front-end.  When I click the logout link, it just takes me here:

http://www.stuffmartusa.com/logout/  and I never actually log out.

My 2nd question is that when logged in, I no longer see my avatar replacing “My Account” link in my menu (shown here:  http://prntscr.com/b7elue).  I know I had this once, and I think there’s a shortcode that I need to put in my menu attributes, but I don’t remember what it was.
