Member page missing advanced search not working

i am building a dating website using the sweet date theme

while building the site I attempted to use the tutorial that is on your help page for Advanced Members Search

i followed all the instructions

1..downloaded and activated Shortcodes unlimited

2..created a members.php file in the sweetdate child theme area in my c-panel

3..I added the code to the file from members-loop.php

4.. I added the css code to the quick css box in the styling section of the theme options in my dashboard

what is happening is there is no advanced search at all and also now when searching members nothing shows up

is there a setting I am missing?

did I do something that disturbed the theme

Any thoughts ?
