I’m unable to recreate the problems you’re having. I edited my main forum page by adding the custom heading, then a text box with the [bbp-forum-index] shortcode. In Theme General Settings I turned on ‘hide the title’, set breadcrumbs to ‘hidden’ and turned on ‘hide information’. I’ve attached a screenshot.
I wonder what’s different between our setups.
