Hola, he creado un código guía para ayudarte con esto, primero, copia el archivo home-search-form.php que hay dentro de sweetdate page-parts, seguidamente crea una carpeta dentro de Sweetdate-Child que se llame page-parts y crea un documento con el nombre home-search-form.php y pega el siguiente código en el (ver adjuntos)
Si usas el formulario mixto, puedes usar el mismo código pero sólo lo que esta entre el div Aplicaciónes.
