What I’m trying to achieve is something like this setup where the forums are broken up:

http://floridastate.rivals.com/forum.asp

I’m looking for groups of forums that have a heading such as “Example1” where “Example1” is not a forum but just a heading.

You can try setting up a forum and then to the “Forum attributes” you can set the Type: Category
Then other forums added should have set the parent to that Forum category.

I know how to set up a forum, I just want to change the layout of the forum.
Now it looks like this:

I’m looking for groups of forums (Categories) that have a heading such as “Romantic”, “Platonic”, “General Discussion”, where “Romantic”, “Platonic”, “General Discussion” is not a forum (Category), but just a heading.

To modify the way it looks you have to modify this file to match your needs: sweetdate/bbpress/loop-single-forum.php
