Change Default Avatar for Members/Groups

How can I change the default avatar for when a member doesn’t add an image to their profile or to a group. Also, do you know what the recommended size of the image should be?

Thank you Catalin, however I know there has to be a way to do it without a plugin. That plugin uses JavaScript to do it and can often cause errors. I’d much prefer just changing out the image wherever that may be.
