Hi,

This is a feature of the BP Profile Search plugin and it is not something we can change for you. If you need assistance with this plugin then please reach out to the plugin author for help with customizing the output of the plugin.

You can show your form on the members directory by going to WP Admin > Users > Profile Search and editing the form you wish to display. Once opened, select ‘Yes’ for the ‘Add to Directory’ and save your form (see screenshot). Make sure to purge your website cache and browser cache to see changes.
