How can I add "Upload photo" to the register page?

I’d like to add an “Upload Photo” and make it a required action on the registration page.

Also, where are the attributes located in the theme which actually dictate what items appear on the registration page?

I’ve looked everywhere but I can’t seem to find where you can add or delete attributes for that page.
