How to add three dropdownlist (single row) in search form?

I want to add three drop down list in the search form. I want to replace search form to registration form with 5 rows
Register Form:
1. single Dropdownlist (gender)
2. Three Dropdownlist (D.O.B) – September / 4th / 1985
3. text box (first name)
4. text box (email)
5. text box (password)

Please help me to include three drop down list in a row and include text boxes.

You need custom development code to achieve this… here is the file that needs to be modified:
