Mixed Form Not Working

You had installed Sweetdate on my site at yeniarkadaslar.com
I have started working on it for some small modifications (like logo etc.) but the search form or the mixed form doesn’t show up on the homepage.
If you want to take a look the wordpress credentials are still active.

Hello, have you setup your home page? Did you select the form in Sweetdate > Homepage?
If yes but still not working, please share admin credentials 🙂

Hello, what changes have you done before the issue started? Did you activate new plugins? Have you tried disabling them? I dont see any change done by me to the search form code
