newsletter no fields appear

Hello i have a problem with the newsletter widget. Fields “name” and “courriel” dont appear, do you have any ideas about whats wrong with my newsletter widget ?

I activated the main theme instead child theme as you mention and I still have the problem. For the newsletter i’m using the kleo widget included inside the theme

If you still encounter the problem even with the default theme it means this has nothing to do with our theme and your problem it’s not caused by sweetdate theme
