Odd Styling of Page

Hi,

We are getting a weird layout since our last update on our membership signup form. I have attached a screenshot. There is a description that normally sits below the variation drop down. Currently it is sitting next to it but squished. Also when the browser window is smaller it creates a lot of white space.
