Premlink is effecting time to time getting 404

Permalink is getting 404 after some time I have to submit the permalink again and it works fine but after some time getting the same issue please help i chq plugin and try twenty twelve there its working fine on that but there is something wrong please support

What are the steps to re-produce the issue? How did you had tested in twenty sixteen VS kleo and you have seed the bug is no more happens?
