register form for other pages

what is the shortcode to add the home page register form on other pages?

I want two separate members a free membership and a paid membership that get access to an online class

here is the flow I am looking for
the home page has the form http://matchdatelove.com/
but through the site I am putting some CTA to get people to register and I want to direct them to this page http://matchdatelove.com/register/ to sign up for free. But there is no form how do I add the home page form to this page?

At the same time I have this page which is marked as member
http://matchdatelove.com/dating-success-university-online-program/
but the button takes them to the free register page and not my membership page
http://matchdatelove.com/membership-account/membership-levels/
how do I redirect this.

One last issue
on the homepage I have a video that I want to have shown when some clicks on the tv commercial button I added the youtube link but the player stalls out

The register button can be changed from PMPRO advanced settings to point to your desired page.
The video link is wrong. If you followed our example you would have seen that is should be like this:http://www.youtube.com/embed/IolkS-ezJxI
