Specific Search Fields

Hi, I’m having a lot of trouble coming up with a sufficient search function. I’ve tried Geo My WP, BP Profile Search ,and a few other plugins, NONE of which are working the way I want them to. What I simply want is a basic search form, pretty much exactly like the one from the Sweet Date Theme. Example: https://realtransrelationships.com/members/

That search bar is perfect. All I need is: “I am a” , “Looking for”, and “State” . And then I need those fields to correspond with the profile fields my users [will] have.

To sum this question up more simply.. is there any kind of short code or something you can give me so that I can have a search function very similar to the one I have on https://realtransrelationships.com/members/ ?
