Hello, it should be: Gravatar – Facebook – Default image
As wordpress always gets gravatar by the email, but im not sure if our facebook setup would be on top of that, as i haven’t tested it. But the order should be gravatar first

Hello, please go to the post that have comments closed, edit it and at the top of the page open screen options and check discussions then at the end of the post edit screen find discussions section make sure Comments are allowed.
Also check at Settings – Discussions to make sure you do not have it set to close after x time

Hello, if registering with facebook it should get the facebook photo, for a normal user that has a gravatar email it gets gravatar and for an user that doesnt have gravatar it gets the default one.
