Hello, the solution is to rebuild it as you did before, i don’t have an idea of why it disappeared, but as you said that in revisions everything was the same, the only way is to rebuild it, try to make the image bigger, add the image carousel as you did the first time 🙂
I can’t help you with this custom request because i do not know exactly how it was, the one that knows how it should be it’s you 🙂

About the other issue, what do you mean by “make the page header shown along with the breadcrumb”

For 1- i do not see the issue, as it’s seems only to appear for you, i can’t do much if i don’t see it, i’m sorry
